"This web application allows enrolled users to conveniently search for patients in the ImpactSIIS Central Registry and to view the patients' vaccination record. In addition, authorized users can add and edit patient records and vaccination records, as well as maintain facility, physician, and lot number data."

If you don’t want to be tracked for even one vaccine — including any future COVID-19 vaccines — you should consider opting out of the whole system. Opting out means that you and your child(ren)’s records will not be maintained electronically, and you won't get vaccination reminders.
